Ministry Questionnaires
Prayerfully consider the ministry that you are interested in serving in. If it is your desire to serve in more than one ministry at a time; please keep in mind those ministries that will not conflict time wise with one another (Example: Parking Lot and Ushering Ministries, etc.).
Please print the "Ministry Questionnaire", which also includes two "Personal References". Complete the Ministry Questionnaire and mail or hand-deliver it to the church Office. Take the two copies of the Personal Reference forms, along with two return envelopes and distribute them to two people who would be willing to provide us with additional information concerning your personal and spiritual character. Assure them that their responses will be kept strictly confidential. We would ask that these two people be Christians, and if possible that they also fellowship here at Harvest.
The coordinator for the ministry you are interested in will contact you as soon as he/she has received both references. We ask that you please write your full name and the ministry you are interested in on each reference and take responsibility for the postage on the return envelopes. This will help to encourage an immediate response and insure that your references are added to your Ministry Questionnaire.
